Medical Disclosure I confirm that I do not have any of the following contraindications: pregnancy, severe hypertension, heart conditions, cold allergies, or open wounds. I understand that I should consult with my physician before using cryotherapy if I have any medical concerns.

Liability Waiver I acknowledge that I have read and agree to the full liability waiver and release form. I understand the risks associated with cryotherapy and agree to hold Recovery Plus harmless from any injuries or damages that may occur during my session.

Informed Consent I have been informed of the benefits and potential risks of cryotherapy. I understand the procedure and consent to receive cryotherapy treatment from Recovery Plus. I will follow all safety instructions provided by the technician.
